Third, justice provides methods and guidelines that when followed, it will further the premises of justice[6].à Take human rights, for example.à If human rights are honored, people will be respected and treated as individuals, and when human beings are treated as human beings, there is justice. Fourth, justice is objective and stands apart from human intervention.à The rules of justice will not be bent because of human volition.à It can stand alone, and must be applied to all.à Subjectivity can only cause injustice.à As was earlier mentioned, justice is universal yet specific. The characteristics of love and justice have been dealt with, and the focus is now on both concepts.à How are love and justice related? Can they exist together? How does love and justice differ? How are they similar? According to Henry, ââ¬Å"love and justice cannot stand juxtaposed.[7]â⬠à It is because love may transcend the very concept of justice, but it cannot be sustained with something less that justice.à Both concepts are related but the relationship between the two cannot be called equal.à It is not equal because the premises of one concept may exceed or fall short of the other. For example, justice can exert power to achieve its desired effect, but love cannot do the same.à As was earlier discussed, love must be voluntary.à It cannot demand results.à It must not resort to force to establish its ends.à On the other hand, justice exerts power to maintain its characteristics.à For one, for justice to subsist in a specific social order, power must be applied. Twelfth Night The complex plot - full of mistaken identities, misdirected passions, high comedy, low tricks, and unexpected poignancy - begins as a ship, carrying the identical twins Viola and Sebastian is wrecked off the coast of a fictional country, Illyria. Viola is washed ashore on this alien coast and becomes convinced that her beloved brother is dead. She learns that she is near the home of Olivia, a young countess who is also in mourning, for her recently dead father and brother. Accordingly, Olivia has sworn to have no contact with men for seven years, and in particular she is rejecting the amorous advances of the young Duke Orsino Desperate to know how to survive, and to keep the spirit of her twin brother alive, Viola decides to disguise herself as a boy. She transforms herself into Cesario, enters into the service of Orsino and is soon sent to woo Olivia on the Duke's behalf. Olivia remains unmoved by Orsino's attentions but finds herself instead attracted to young Cesario, who in turn begins to fall in love with Orsino. As Viola says, My master loves her dearly; And I, poor monster, fond as much on him. And she, mistaken, seems to dote on me. When Viola's twin Sebastian suddenly emerges, alive and well, this triangle is complicated almost to a breaking point. While Orsino, Viola, Olivia, and Sebastian are preoccupied with their romantic destinies, Olivia's household is equally occupied with a power struggle between the ill-tempered, repressive steward, Malvolio, and her boisterous and bibulous uncle, Sir Toby Belch, accompanied by his vacuous, misfit friend, Sir Andrew Aguecheek, and Olivia's maid, Maria. For his own financial ends, Sir Toby encourages Sir Andrew to woo Olivia, while plotting Malvolio's humiliation. Throughout, Feste, the most enigmatic of entertainers, comes and goes between the two households, sparing no one he meets his barbed insights and his wit. Written in 1600 or 1601, this play is Shakespeare's last romantic comedy. The comedies which followare much darker. Twelfth Night embodies many of the themes from his earlier comedies. For instance, he employed the device of having a woman fall in love with another woman disguised as a man. In some ways, Twelfth Night can be termed Shakespeare's apology to Phebe. Whereas Rosalind dominates As You Like it, we have many centers of interest in Twelfth Night. Orsino continues the theme of overly idealistic love we have seen in Orlando. The setting has a different feeling from Merchant of Venice and As You Like It--two plays in which Shakespeare clearly contrasts the world of reality with some more tranquil environment. Instead (rather like the single setting of Much Ado About Nothing), we see in Twelfth Night the twin centers of power--the estate of Orsino and the estate of Olivia. Illyria itself is a very different place from the Forest of Arden, and before the play began, Antonio has been in battle against the forces of Orsino. The interpretation of Malvolio will be crucial to your overall response to the play. I would observe that his appearance cross-gartered with yellow stockings almost inevitably delights audiences. But how much sympathy do you give to Malvolio? Unlike Jaques and Duke Frederick, he is an unwilling scapegoat. I propose that the older the actor playing Malvolio, the more seriously and tragically the audience will take him. It causes a blocked or reduced flow of urine which can result in a range of manifestations, from asymptomatic to severe discomfort. Moreover, it can lead to serious complications such as urinary infections and renal insufficiency secondary to urinary retention. Blunt perineal trauma, urological instrumentation, chronic inflammatory disorders such as lichen sclerosus et atrophicus and sexually transmitted diseases are the most frequent causes of strictures; a large proportion are iatrogenic . Currently, three different interventions are used to treat urethral strictures: dilations, optical internal urethrotomy and open urethroplasty(1). Dilations are easy to perform in every day clinical practice, however they show the highest recurrence rates while their outcomes are the less satisfying to the patients. On the other hand, open urethroplasty shows the lowest recurrence rates and its outcomes are the most satisfying to the patients (2). Although it is the current gold standard against which the traditional treatments are compared, this technique requires skills, expertise and equipment, often not available in the resource limited settings. For the above reasons, most patients with urethral stricture are offered optical internal urethrotomy . This procedure is preferred as the first treatment option by many urologists, as it is performed within short operative times, under either spinal, or local anaesthesia. It can be also done as an outpatient procedure for the treatment of short urethral strictures . Optical internal urethrotomy is performed by a rigid urethrotome or a flexible cystoscope/urethrotome combination. The instrument is inserted and guided to the face of the stricture and a small blade at the tip of the instrument is deployed using a trigger mechanism to cut the stricture at locations determined by the surgeon. Upon completion of the internal incision(s), the instrument is withdrawn and an appropriately sized Foley catheter is inserted through the repair and into the urinary bladder. Hospitalization lasts 2 days and the patient keeps the catheter a few days . However, technical difficulties associated with poor visualization of the stenosis or of the urethral lumen may increase procedural time and substantially increase the failure rates of internal urethrotomy. In this report we describe a technique for urethral catheterization via a suprapubic, percutaneous approach through the urinary bladder. Case Report A 35-year-old male patient, presented to our urology department with a severe (> 5 cm), tortuous stricture of the penile urethra, previously diagnosed by descending cystourethrogram (Figure 1). A suprapubic catheter was in place. Technique The urinary bladder was filled with 300 millilitres of diluted iodinated contrast (contrast/normal saline: 1/3) via the suprapubic catheter. A 0035â⬠, J-tip standard angiographic guidewire was inserted into the urinary bladder through the suprapubic catheter. The latter was removed and exchanged with a short (11 cm), 5-French angiographic sheath. The angiographic guidewire was subsequently withdrawn and a Cobra-1 angiographic catheter (5 Fr, 65 cm) loaded with a 0035, hydrophilic, J-tip guide-wire was inserted into the bladder. Under fluoroscopy, the catheter-guidewire combination was guided towards the internal urethral orifice and urethra was catheterized. The hydrophilic guidewire was used to negotiate the stricture and was finally advanced through the external urethral orifice (Figure 2). The angiographic catheter was subsequently advanced through the stenosis, over the guidewire (Figure 3). The hub of the catheter was cut off and the vascular sheath was removed. The posteri or part of the catheter is stabilized on the abdominal wall with a suture. The patient was then taken to the operating room and placed in lithotomy position, under spinal anaesthesia. By keeping the angiographic catheter stretched, the rigid urethrotome was inserted in the urethra (Figure 4) and directed to the face of the stricture. The blade at the tip of the instrument was deployed using a trigger mechanism to cut the stricture at the 12 o clock location along the entire length of the stenosis. Upon completion of the internal incision, the instrument was withdrawn and a Foley catheter was inserted through the repaired stricture and into the urinary bladder. Results Antegrade catheterization of the urethral stricture and subsequent treatment of the stricture by using the angiographic catheter as a guide for the urethrotome proved feasible. No urethral injury, false passage or development of fistula were observed. The fluoroscopically guided antegrade catheterization of the stricture lasted 11 minutes. The patient remains free of recurrence 7 months post-operation. Discussion Internal urethrotomy has advantages of ease, simplicity, speed and short convalescence. However, success rates vary and long term results are generally low. In the short-term (less than 6 months), success rates are 70 to 80 percent. After one year, however, recurrence rates approach 50 to 60 percent and by five years, recurrence falls in the range of 74 to 86 percent(4). Although different studies have proposed different etiologies as poor responders to optical internal urethrotomy, technical and anatomical factors such as reduced visibility during the operation and stricture length are uniformly recognised as predictors of recurrence . Other factors associated with treatment failure are the perioperative urinary infection, the presence of periurethral fibrosis (spongiofibrosis) and stricture etiology(5). In cases of severe urethral stenosis or excessive urethral tortuosity the actual site of the stricture may not be visible at all4. Intraprocedural bleeding, presence of blood clots, inflammatory or injured tissues add extra difficulty in localization of the stenosis and in safe advancement of the urethrotome. Retrograde instillation of methylene blue through the suprapubic catheter and/or antegrade advancement forwarding of an ureteral stent are usually used in order to visualise narrowed urethral lumen, or to guide the urethrotome through a tortuous urethra respectively. However both manoeuvres are of questionable effectiveness. In several cases the procedure is concluded in two sessions; in other cases a scar appears after urethrotomy (probably as a result of a false route of the urethrotome) and this causes a new stricture. The above may explain surgical success rates as low as 20% . To overcome these problems, we devised a technique which is based on percutaneous, trans-cystic catheterization of the urethra. The technique requires low-cost, widely available materials of interventional radiology and a fluoroscopy unit. The procedure is brief, well-tolerated and requires no anesthesia. Our initial experience showed that even a long and severe stenosis could be negotiated with rotational and gentle probing movements of the hydrophilic guidewire. Of note, we preferred to utilize the angiographic catheter instead of the guidewire as a guide for the urethrotome; compared to the guidewire, the angiographic catheter proved more stable, more efficient for straightening of the urethra and more easily visible endoscopically. It is now that our political leaders and decisions have made up for what they were accounted for and have provided the back-bone of America today. Our government is what establishes America's decisions. Although some consequences can be made from what their previous aspects were, and how some of the decisions have made distrust in the government from Americans, it is only with said that, ââ¬Å"Their failure to act, their failure to make decisions, is itself an act that is often of greater consent ounces than the decisions they do make. It's not necessarily the cause and effect of their decisions, but the act f speaking for America and doing something about the complications that arises within our world and our country to do the best they can to provide security and freedom even though there are limitations to freedom for security. Mills has stated, ââ¬Å"The assumption that the security of the nation supposedly rests upon great secrecy of plan and intent. Along with the statement, ââ¬Å"With the wide secrecy covering their operations and decisions, the power elite can mask their intentions, operations, and further consolidation. â⬠I believe Mills is pointing out the directive of our national security being in the hands of the political officials because the government is the elite officials of America and has the authority to reach beyond the limits of other powers. Some may say the military is the elite of America because of the power and figure they portray.One may believe that the military is the most elite because they have more representatives, leaders and followers than those of our political leaders. One may obtain the fact that the military protects and has the secrecy of the united States in withstanding war power, but this is not accurate concerning the fact that the government has authority over the litany's actions. Mills has stated, ââ¬Å"The military order, once a slim establishment in a context of distrust fed by State militia, has beco me the largest and most expensive feature of government.. Thus proving my element for elite power containing that the military is not established without the governments control and authority to make decisions for the military. One may believe that corporate has the most elite power over all, but corporations only have financial power not power over the decisions that are made within our country financially. Our political establishments indeed are ended by corporate, but they cannot make the decisions nor have the authority to do what the government can.The government tells corporate where to spend their money and what they can spend it on. ââ¬Å"The decisions of a handful of corporations bear upon military and political as well as upon economic developments around the world. â⬠Mills has stated which is proof along with my statement on economic decisions going through the government before corporate is able to take action on what they are deciding to do. In conclusion politic al officials are the most elite of the United States today.My opinion is not disregarding the other elite powers Mills has claimed above, but only withstanding that political power is the most elite out of the two other objectives. Nothing can go forward without the government's approval and action to speak for Our country.
